Live Well. Be Well. Community Health Education

Live Well. Be Well. is a community health program led by a Registered Nurse, a Registered Dietitian and a Health Educator.

Live Well. Be Well provides:

  • School Health Education
  • Individual Health Coaching
  • CPR Classes - Adult/Child and Infant
  • Monthly Mall Walks (with optional blood sugar screenings before and after) 
  • Diabetes/Prediabetes Workshops
  • Grocery Store Tours
  • Fun Family Events throughout our Community

Living With Diabetes

The group meets on the third Wednesday of the month (excluding July, August and December) at the Monroe Home Care Shoppe to discuss diabetes management and other health topics.

Individuals with prediabetes, diabetes and support persons are welcome to attend!
